Our hospital discharge support project can help!
If you meet the following criteria, please get in touch with our coordinator Kathryn:
. Live in CH64
. Aged 55 and over
. Currently in Arrowe Park Hospital/ Clatterbridge
. In need of reassurance and low-level support with your discharge
Our Hospital Discharge Support team can help you return home smoothly, providing reassurance and practical support such as:
. Meeting you at your home when you are discharged to check on your wellbeing
. Do a small food shop
. Signposting and referring you to relevant services and activities for any ongoing support that can improve your wellbeing.
Our aims are:
. To increase independence
. Improve wellbeing
. To reduce a delay in discharge and readmissions for social reasons.
